Background: Subperiosteal hematoma is the accumulation of blood between the periosteum and the skull, which commonly occurs in the neonatal period but rarely in other ages. Subperiosteal hematoma can be self-absorbed in most cases and occasionally may cause ossification. Subperiosteal iliac hematoma may present acutely in adolescents or chronically in asymptomatic adults. Clinical presentation. Acute: in adolescents , usually with history of recent fall, giving complaints of pain and limping due to crural nerve compression. Chronic: incidental finding in asymptomatic adults. Imaging findings Orbital subperiosteal hematomas (OSPHs) are less common; occurring primarily in young males as a direct result of craniomaxillofacial trauma, although the development of spontaneous OSPH has been reported.

It is a rare condition which occurs following severe damage to subperiosteal blood vessels, mainly subsequent to facial traumas and most commonly in male children. 1,2 Less frequently, it appears secondary to extension of subgaleal hematomas, after surgeries, 3 or with involvement of Orbital subperiosteal hemorrhage is rare, with most cases occurring in young males as a result of direct facial or orbital trauma.
